I guess it will largely depend on what you want to do with it. I have mostly been playing with android for the time being using a regular large PI heatsink (see attachment) and had no issues what-so-ever. I know however that fire219, Xalius and others have tested pushing the SOC past the base-specs (see ayufans git for more info). For this sort of application you will need something beefier - fire219 even had a fan on his. So, if you want to do media in android or run OMV then a regular Pi heatsink ought to do (perhaps even no heatsink at all); other than that, you may just want something a bit bigger and better.
