If anyone is interested, this has been resolved. I found this issue on the mmsd-tng gitlab page today:

Every attempt to retrieve the MMS results in a 503 (AT&T USA) - manually curl'ing works

This was precisely my problem. Version 1.8 of mmsd-tng fixes the issue. I have it running on my phone now and have been able to receive MMS messages.
